When you submit an enquiry or book an NDIS Exercise Health Check, we collect the details you provide: your name, phone number, email address, and information about your NDIS plan and support needs that you choose to share with us.

How we use it

We use your information to respond to your enquiry, book and deliver our services, and communicate with you about your care. We do not sell your personal information, and we do not share it with third parties except the service providers we use to run our practice (such as our booking, practice-management and communication systems) or where the law requires it.

Health information

As an allied health provider, we handle health information in line with the Privacy Act 1988 (Cth), the Australian Privacy Principles, and our obligations as a registered NDIS provider. Clinical records are stored securely in our practice-management system and accessed only by the team involved in your care.

Access and questions
